For best results, be sure to do the following before creating a table of contents for a book:
Before you create a table of contents, verify that the book list is complete, that all documents are listed in the correct order, and that all headings have been formatted with the appropriate paragraph styles.
Be sure to use paragraph styles consistently throughout the book. Avoid creating documents with styles that have identical names but different definitions. If multiple styles have the same name but different style definitions, InDesign uses the style definition in the current document (if a definition exists there), or the first occurrence of the style in the book.
If the necessary styles do not appear in the pop‑up menus in the Table of Contents dialog box, you may need to synchronize the book so that the styles are copied to the document containing the table of contents.
There is a known bug in InDesign CS3 that occurs at times when generating a TOC in a book. The last character in the last entry of the TOC is sometimes deleted. I'm I correct that I can't make a table of contents from chapter titles located only on Master pages? What good are Master pages if you can't make TOCs from them? Where should I put the document name?

Yes, it's correct that you can't make a ToC from chapter titles located only on master pages. It makes sense to me because master pages can be (and usually are) applied to multiple pages. If you want a chapter title on a master page to appear in a ToC, simply override the master item on the page where master is applied. To do this, hold down Shift+Ctrl/Command and click the dotted text frame on the document page that contains the chapter title. Is there anyway to ignore some pages on the table of contents?
For example, when we have several pages with the same title, instead of having the same title repeating for every page it appears on, to appear just once??

One possibility is to create a second identical style. For example, if you use a "Heading1" style to apply to the same title on several pages, create a "Heading1B" style that's based on the Heading1 style. Use the Heading1 style in the TOC. If I set up a book with a number of diiferent documents that each use the same template can I use the Section Markers or Chapter Name variables (and their paragraph style) to produce a table of contents or do I have to overide every page ?

It depends. I'm not sure how you want to use them. If the chapter number variable and section marker appear in text that's formatted with a paragraph style, yes, the chapter number and section marker will appear in the toc as long as the paragraph style is included in the toc. However, if you want the chapter number variable or section marker to appear as a page number prefix in the toc, such as "Animals...1-1," "Plants 2-1," and so on, you must use section prefixes.
Hi,
I'm creating a contents page and I would like to ask how can I keep the numbers at the end of the tab leaders as a constant font while the content headers are changed to various font styles like bold and italics?
My current problem is that whenever i edit my content headers to have bold or italics font styles... the numbers at the end of the tab leaders also adopt the bold or italics. I would like to know how to maintain those numbers as a constant font.
Your help is greatly appreciated.

You can apply different character styles to the page number and the dot leaders, so I'm not sure why this problem is occurring. In the Table of Contents dialog box, click More Options. For Page Number, specify a style, and for the "Between Entry And Number" option, specify a different style.
