AS3, like AS2, sometimes fails to fire the init event when load progress is complete. Unlike AS2, when init fails to fire, complete also fails to fire. In AS2 projects an initializer movieclip, placed in the topmost layer to callback to a loader, in a bottom-up loading fla, worked-around the bug. In an AS3 only project, with no fla structure to hold an initializer movieclip, I'm guessing that one sure way to overcome the intermittent failure of the init and complete events is to dispatch them manually if progress event handler reveal that all bytes of the target have loaded. In my samples where init and complete intermittently failed to dispatch, the progress event always revealed all bytes had loaded.

It should be noted that the COMPLETE and INIT events dispatched are different then those on AS2's MovieClipLoader class though they have similar names. (onLoadComplete and onLoadInit)

With MovieClipLoader onLoadComplete would be called after the file was loaded but not yet initialized and onLoadInit would be called only after after its properties were initialized and available.

With the new Loader class in AS3, INIT is dispatched first when the loaded item's properties are initialized and available and COMPLETE is dispatched after.
